Acknowledgment and Assumption of Risk:

I, the undersigned, acknowledge that I am voluntarily participating in activities at St Marys National Indoor Sports, located at 20 Forthorn Place, St Marys 2760, and I am aware that these activities involve physical exertion and may carry some inherent risks. I understand that participating in sports and recreational activities can result in injuries, including but not limited to, sprains, strains, fractures, and other types of injuries.

I acknowledge that the risk of injury may arise from my actions, the actions of other participants, or the conditions of the facility. I assume all risks associated with my participation in these activities and agree to hold harmless St Marys National Indoor Sports, its owners, employees, agents, and representatives from any and all liability, claims, demands, and causes of action, whether known or unknown, arising out of or related to my participation in these activities.

Waiver and Release:

In consideration of being allowed to participate in activities at St Marys National Indoor Sports, I hereby waive, release, and discharge any and all claims, liabilities, and demands against St Marys National Indoor Sports, its owners, employees, agents, and representatives, from any and all injuries, damages, or losses that I may sustain as a result of my participation in these activities, whether caused by the negligence of St Marys National Indoor Sports, or otherwise.

Medical Consent:

I understand that in the event of an injury or medical emergency, reasonable efforts will be made to contact the emergency contact listed above. However, if I am unable to communicate or if the emergency contact cannot be reached, I hereby consent to the administration of any medical treatment deemed necessary by medical professionals.
